Butterfly Survey

If you or your organization are currently involved in a butterfly conservation, education, or research project, please contribute your information below.

If you or your organization are not involved in any butterfly projects, simply fill out the “Principle Contact” section of the form and select “No Projects” in the “Project Type” section of the form.

INSTRUCTIONS:

Please fill out and submit one form for each butterfly project you are currently involved in, have done in the past ten years, or are planning for the future.  If a project has more than one component, please fill out a separate form for each component.  For example, if you have one project that includes both a conservation component and an education component, fill out and submit one form for the conservation component and one form for the education component.  If you have any questions concerning the form or Cleveland Metroparks Zoo’s efforts to support the AZA Butterfly Conservation Initiative, please contact:

Dr. Kristen Lukas at  kel@clevelandmetroparks.com or Kym Parr at  kap@clevelandmetroparks.com.

Thank you,
Kristen E, Lukas, Ph.D.
